CCAP Appointment Checklist
Applicants should bring the following verification to appointments for the Child Care Assistance Program (CCAP).
Applicant Information
____ Driver’s license, personal ID, birth certificate to verify name and dates of birth of adult household members
____ Social Security Cards for adult household members (optional)
____ Proof of legal guardianship if adult requesting care is not related to child
____ Proof of residence & household size – lease, bill, etc. Residence Household Form (DCC90C) – completed by someone other than a member of the household.
Employment and Income Information for All Adults in the Household
____ All pay stubs received in the last two full calendar months (i.e. 8 or 9 stubs if paid weekly; 4 or 5 stubs if paid bi weekly; 4 stubs if paid semi monthly.). For example, if appointment is in October, bring all income information for August and September. For eligibility, income verification must show working at least 20 hours per week.
____ Verification of Employment letter if new employee and have not received 2 FULL months of pay stubs yet (bring letter from employer with number of hours to be worked per week, rate of pay, and start date and all stubs received).
____ Documentation of ANY additional income (examples include, but are not limited to: other job, proof of tips received (tip log), KTAP, SSI, child support, Social Security, recurring cash contributions, etc.) If self employed, bring most recent tax return records (i.e. schedule C, CEZ, F, 1120S, etc.)
Education
____ Copy of current class schedule or letter from training facility that includes name, class days, and times.
____ Students must also be working at least 20 hours per week (see Employment section above for documentation to bring) If participating in unpaid student teaching, internship, or practicum, provide letter from educational facility to document number of hours and days per week and start and end date.
Child Information
____ Social Security Cards for all children in the home (optional)
____ Birth Certificates for all children in the home who will be in child care
____ Immunization Certificates for all children needing care
____ If child has special needs, bring verification (i.e. SSI award letter, for child under 13 years, a statement from a health professional or court if child is 13-19 years.)
Child Care Provider
____ Name, address and phone number of child care provider
