Child Care Service Agreement and Certificate by Email

You can now request to have your client certificates emailed to you on a regular basis. Receiving the certificate by email will give you valuable client information such as start date, co-pay, and child’s schedule right when it is created. If you opt to receive the certificate by email it will not be mailed to you. You will need to print the certificate and return it with your signature and the client signature in order for payment to be made. If you would like to receive your certificate by email please contact Kim Chesser at kim.chesser@ky.gov for Jefferson County providers. Providers outside of  Jefferson County should contact Annette House at annette.house@ky.gov.
